Your First Visit

When you come for your appointment please remember to obtain and bring the following:

• Referral letter from GP, physician or other doctor
• Medicare card, DVA card, pension card
• Private health fund information
• Test results e.g. blood tests, x-rays, CT scans, biopsy results
• Any other additional information which may be relevant e.g. previous treatment and/or operation reports, list of current medications (often included in referral letter)
